The first weekend some metro Phoenix bars were allowed to reopen again, a few businesses were already forced to shut down after violating coronavirus guidelines.
The guidelines, set by the state Health Department, are intended to prevent the spread of COVID-19, the disease that has claimed more than 5,000 lives in Arizona.
Gov. Doug Ducey ordered bars to close at the end of June following a spike in COVID-19 cases. Then on Thursday, Aug. 27, bars and nightclubs that serve food in Maricopa County were allowed to reopen again because the county reached the “moderate” spread category of COVID-19.

To reopen, businesses had to sign an online form agreeing to comply with safety requirements, which include limiting parties to no more than 10 and closing dance floors. The Health Department is asking the public to report violators by calling the tip line at 844-410-2157 or online at the Arizona Department of Health Services Business Compliance page.
Bars that have been shut down can reopen after getting the OK from the Health Department. Since the Aug. 27 reopening, here are the bars and nightclubs in metro Phoenix that have been forced to close and had their liquor licenses suspended indefinitely:
Glow Shots and Cocktails
The Mill Avenue bar in Tempe received a notice on Aug. 30 from the state Health Department. The bar was not operating as a dine-in restaurant and was not enforcing social distancing, keeping people seated at tables or prohibiting dancing, ADHS told The Arizona Republic on Aug. 30.
The Republic reported that the nightclub shared a video on its Instagram account showing people who appeared fairly distanced inside with the caption, "The liquor board shut me down for this. 25 percent occupancy with food. 10 people standing is what did it!"
Casa Amigos and Skylanes
Casa Amigos, part of Evening Entertainment Group, is a restaurant and nightclub in Old Town Scottsdale. It has a bowling alley upstairs called Skylanes, which appeared to also be included in the closure.
The Department of Liquor Licenses and Control said investigators on the Friday after reopening observed violations related to distancing, mask-wearing, dancing, standing and table occupancy, The Republic reported Aug. 29.
In May, Casa Amigos was among the Scottsdale nightclubs that were given a notice to comply with social distancing and mask requirements.
Bottled Blonde
Bottled Blonde, part of Evening Entertainment Group, is a pizzeria and nightclub located next to Casa Amigos in Old Town Scottsdale. The club has another location in Dallas and one planned in Houston, according to its website.
As with Casa Amigos, investigators on Friday observed violations related to distancing, mask-wearing, dancing, standing and table occupancy, The Republic reported Aug. 29. Bottled Blonde was also among the Scottsdale bars in May that were given a notice to comply with social distancing and mask requirements.
